The Pathway 3 model refers to the discharge from hospital of a person who is medically fit to 'step-down' care, such as in a care or nursing home. During December 2021 and the early months of 2022, we explored how well the discharge to assess, Pathway 3 model was working, and what changes could be made to improve the standard of NHS care for everyone.
